Where are your clinics located?

Cucamonga Valley Medical Group has 6 office locations. Please visit our locations page for addresses and hours of operation.

What insurances do you accept?

We accept PrimeCare/Optum, Inland Faculty (IEHP and Molina), Most PPO’s and Medicare. If you are not sure whether your insurance is accepted or not, please call 909-429-2864.

Where can I complete my lab work?

Lab work can be completed at PrimeCare and Optum Care.

What should I do if I lost my lab orders?

If you lost your lab order, you have access to a digital copy on the healow® app. If you do not have a healow® profile, you will need to create one to obtain your lab order. If you do not have the healow® app or are having trouble accessing your lab order, please call 909-429-2864.

How can I transfer or get a copy of my medical records?

To transfer or receive a copy of your medical records, please fill out this form and mail it to our North Fontana Office located at 16465 Sierra Lakes Parkway, Suite 300, Fontana, CA 92336. Please allow 5-7 business days for our Medical Records representative to address your request. If any additional information is needed, our Medical Records representative will contact you via phone.

How soon should I request a medication refill and who do I call?

Medication refill requests should be submitted 5-7 days before your last allotment of days. Please call 909-429-2864 and speak to a representative in our Medications department.

When can I get a flu shot?

Flu vaccines typically become available in mid-September. Once we have received our flu vaccine doses, patients can make appointments to receive their flu shot.

When should my child be vaccinated?

The Centers for Disease Control and Prevention (CDC) recommends staying up to date on all vaccines. The timing of when vaccines are administered is dependent on the patient’s age and health history. To know if you or a family member is up to date on all recommended vaccines, ask your provider or check your patient portal on the healow® app.

What is a DO, MD, NP, and PA?

Our Doctors of Osteopathic Medicine (DO), Doctors of Medicine (MD), Nurse Practitioners (NP), and Physician Assistants (PA) work collaboratively to educate patients on their health and find ways to manage it. Our providers practice preventative medicine and approach their methods using a holistic perspective, focusing on all aspects of the patient’s body, mind, and soul.

What services does the healow® app offer?

The healow® app offers a variety of tools to help keep patients informed and organized. The app provides you with immediate access to your health information, such as your listed allergies, medications, lab orders, and vaccinations. Additionally, healow ® offers a safe way for you to meet with your provider using their TeleVisit feature. All your health, all the time, at the tips of your fingers.

How do I obtain my referral once it has been approved?

Please contact our main number to speak to our referral department. We would be happy to email your referral to you via email encryption or mail it to your home address.

I received a bill or statement from CVMG; who can I speak to about it?

If you received a bill or statement from Cucamonga Valley Medical Group and the return address on your statement is 299 West Foothill Blvd., please call 909-429-2864 and speak to our billing department.
